Forms - Formatting and Email Confirmation

I want to use a form to review website updates and send the review results to the corresponding project manager. Since I am reviewing approximately 40 websites with 40 different project managers, I have set the form up to email the project manager the form responses using the "send me a copy of my responses" feature. I chose to send the email this way because I want the project managers to auto receive their data only, preferably in a prettier format. The issue is the form headings and descriptions are not included in the confirmation email. Is there a way for the confirmation email to send the form headings and descriptions in addition to the responses?

Lastly, I wrote a custom confirmation email message but it is not appearing in the confirmation email. Why is that?
