Form Query String - Changing Hyperlink
I need to create a form that will have a unique, clickable URL code. I can successfully autofill a section of the form to contain this hyperlink but it is not clickable since it is an editable field (see example below). Any ideas?

You're correct, because this is a field that the form user could potentially fill out, it's set as a text field and will not be a clickable link.
I would suggest using a Header/Description section in the form instead of using a field associated with a column/cell in the sheet:
You could also add a description to the "Link to Images" field instead. See: Add Additional Elements to a Form
