Form Entry Metrics by Category

I'm creating a metrics sheet that will have data input by external parties via a form. I'm trying to think of a simpler/more user friendly way to display categories on the sheet and form.
There are many category options and I need the # of occurrences entered for each category to tabulate monthly metrics. Most of these categories will have no occurrences, while several will have many. The only way I can currently think to display this information in my sheet (and form) is to have a column for each category I.e. "# of Occurrences of Category A" (and so on through Z). And then there would be a line item in the form for each category, most of which would be left blank, but several will have entries. Is there a way to simplify this in the form and sheet such that all the categories are available, but only the relevant ones are displayed (when selected)?

I would do a multi-select dropdown that has all of the categories as options. You can then use field logic in the form to only show those category fields based on what was selected in the dropdown.
