In control center, when I fill out the project intake form, it isn't creating a new project.

I just went through the process of creating a blueprint in control center, but whenever I fill out the intake sheet form to create a new project, it does not create a link in the intake sheet, or create a new project. Even after I click the approval box. When I go to create it in control center, this box popped up

I am not really sure what to do from here. Back in october I was able to run it and it worked (that is the first test you see). But now there must be a setting that is different? Is this a simple thing I am missing?

@bilism_kaley is that this? And if so, I tried it with this on and it had the same issue

@bilism_kaley, I would look at the Intake Settings step when you go through the Blueprint wizard. Does it look something like this? If there is incorrect mapping in this step, it may result in the issue that you described.
