Creating a form

I need to create a form that allows folks to request materials from inventory; part, description, quantity, etc. when they submit the form it should then go to the Materials team to fulfill. how do i set the form up so that they can select multiple parts/quantities on the same form with out having to submit one per item? probably something simple I'm missing, but this is my first form in Smartsheet. thanks!

You would have to have multiple columns set up. Part 1/Qty 1/Part 2/Qty 2/Part 3/QTY 3/etc.
