Forms with Checklist

Hi!

I am creating a form that contains checklist for a newly hired employee.

The logic is that, when he check the box, he gets to upload a file or document


Can someone help me with this?

    I hope you're well and safe!

    To add to Jeff's excellent advice/answer.

    Unfortunately, you can only have one File Upload section, but you can add multiple files.

    I'd recommend adding checkboxes or a multi-select column type where the submitter can select each attachment they've added.
