Automated Workflows for Appointment Reminders

I am currently working on a Solution for a Pilot program that three of our Pharmacy Sites are participating in. This Solution will contain 4 forms and 3 Scheduling Calendars, One of the "asks" was to be able to send an appointment reminder to the individual scheduled.

I have set up an Alert as the Appointment reminder and have the automation settings set to Unrestricted but when testing it with 3 outside emails I still get the Workflow Delivery Alert. My assumption with the unrestricted setting is that "Any email referenced in a workflow" would send to those email address with out adding these to safe sharing list which would be impossible as we would not want these outside individuals to have access.

I have also checked the Account Level Permissions since I am the System Administrator :)

Has anyone been successful with this type of workflow?
