Developing a Matrix Questionnaire

Hello,

I am hoping someone can help me design this type of Survey Form to send out to my work employees. I would like to ask them to pick an answer (that I give them), by selecting the radio button. It seems easy enough however, I do not see this. I have attached a screenshot. If you know how to do this, I would really appreciate it.

You can get it kinda like that. I've posted below. Just make dropdowns and in the display type section of the web form editing tool select vertical or horizontal. I picked vertical because horizontal makes it overlap.

Thankyou for your reply. I too was looking at it but it looks so messy when you have only two answers and about 30 differing questions that need to be answered either one or the other.

For now I have gone with qualtrics and will import the data into my smartsheet when my survey comes back.

Dynamic View edit button missing
The edit button is missing from the right side of the dynamic view that I am the owner of. This is a recent development as I have edited this dynamic view previously.
