PDF form fillable fields gone when using DocuSign for generating document???

Hi!

So I've been playing with the document generator from row data. I created a fillable pdf form and was able to generate the document from row data with this form. Now, my form requires signatures, so I would like to use the DocuSign mapping option for this. I've added my fillable pdf form as a template in DocuSign, but when I get to the mapping part, it will not let me map the fields on the form. It looks like my fillable fields are gone?? See screenshots below. Thanks in advance for any help that can be provided!!

I have only had luck using Adobe for creating fillable forms for the document builder.
